Forms For Classes

You will receive digital forms for most classes. That is the preferred method of completeing forms.

You may print, complete, and sign these paper forms for the class if needed. Once signed, email them to info@divingenterprises.com along with your personal information in the email.  You may scan or send a picture of the forms.  You MUST bring the originals to the store. If you choose to print these forms we have no way to verify your eligibility until we receve the copies or originals.
